WELCOME TO THE CONFIDENTIAL
UNIVERSAL WELLBEING EVALUATION TOOL (UWET)
The Universal Wellbeing Evaluation Tool (UWET) has been developed to support people and organisations to evaluate the status of the 70 variables that influence Universal Wellbeing.
This UWET has been designed to i) help people get the supports they need faster,
ii) recognise strengths iii) prevent the consequences of poor Universal Wellbeing, and
iv) empower people to THRIVE.
IMPORTANT
The information in your UWET Report is Confidential, only you and your Accredited Universal Wellbeing Facilitator can see it.
Your UWET Report cannot be disclosed to any other person without your written permission.
2. Answering each question as accurately as you can, will improve the value of your UWET Report.
3. You will be asked to give your Consent to having your UWET results included in research we are conducting (without you being identified) at the end of the UWET.

We would really appreciate you saying YES as it will help us to keep improving the
UWET and supports for Universal Wellbeing.
Ethically approved Research Information Sheet and Consents for Adults and Minors
(those under 18 years) can be opened below for you to read and or download.
4. Terms
o Ethnicity refers to your unchangeable genetic group (or groups). For example: Scottish, Scandinavian, Aboriginal.
o Culture refers to the way you and or your family chose to live. For example: as a Christian, Vegan, culturally based Cook Island or Deaf.
o Spirituality includes your beliefs and values, these may or may not be religiously based.
o Intuition is a sense that something is right or wrong, or about to happen.

Family & Whānau Universal Wellbeing Checks
Would you like to thrive rather than just survive?
Would your family like to enjoy better wellbeing? Our Family or Whānau Universal Wellbeing Checks can evaluate the 70 key factors that could be impacting your Family or Whānau wellbeing. Call or email for an appointment and one of our professional wellbeing facilitators will guide you through a Family or Whānau Universal Wellbeing Check.
After your first appointment your facilitator will take you through your Family or Whānau Wellbeing Check Report, this will help you identify what is going well and where your Family & Whānau may be facing wellbeing challenges and changes could help.
A Wellbeing Check includes
1. Universal Wellbeing Literacy Support
2. A Family or Whānau Universal Wellbeing Check
3. Provision of a Family or Whānau Universal Wellbeing Report
Investment:
• $700 Inc. GST (Family & Whānau Universal Wellbeing Check for up to 4 people)
Wellbeing Check Options
You can have your Universal Wellbeing Check and report back face to face, online, or by telephone
Duration:
A Family or Whānau Universal Wellbeing Check takes 3 to 4 hours
A Family & Whānau Universal Wellbeing Check Report back takes 120 minutes
